Booking Form

Places on upcoming chapters of the Bladelands Chronicles will not be booked until we have received payment, and every booking must be accompanied by a copy of this form. For details of payment methods, see the Payment Information document. Increased charges apply to payments received less than 2 weeks before the event.

Places in each chapter will be generally offered on a first come first service basis. Once your payment has arrived you will receive confirmation of your booking - if you have not received this within 2 weeks of making your payment please contact us in case there has been a problem. Should the event already be full when you send your booking form and payment we will place you on a waiting list and will contact you if we have any cancellations. If we do not have any cancellations we will return your payment to you, or credit it towards a future event.

Cancellation policy: If you make a cancellation made up to 1 month before the event we will refund your payment in full, or credit it towards a future event. If you make a cancellation up to 2 weeks before the event we will refund 50% of your payment, or credit it towards a future event. Cancellations made less than 2 weeks before the event are not eligible for a refund.

The person/people you are booking for:		When booking for multiple people, please separate details with commas, and ensure you give details in same order (Eg: Name 1, Name 2; Email 1, Email 2) or otherwise clearly indicate which player details refer to.
Player(s):		Real name(s) of everyone you are booking for (including yourself).
Contact Details:		Email (preferred) or telephone for each player, where possible.
Date(s) of Birth:		Please contact us before booking places for people under the age of 18.
Character(s):		The name of the character each player intends to play at this/these event/s. Don't forget to send us an up-to-date copy of your character sheet before the event.
Allergies:		Food, medication, makeup or other allergies.
Phobias:		Things to which you would not like to be exposed during the game.
Medical Information:		Existing medical conditions, medication being taken, etc.
Other Information:		Any other information you think may be of help to us.

~~Volume 2, Chapter 9~~ 10-12 July 2009:	CANCELLED!	Due to circumstances beyond our control (a double booking by the people at Fort Widley) this event has had to be cancelled.

Payment:
Method of Payment:		Choose the method of payment you will be using. For details of payment methods, see the Payment Information document.
Payment Date:		Give the date you sent or intend to send payment.
Payment Details:		Give the name of the originating account (for bank transfers), the location and sort code of the branch you paid in at (if paying directly into our account), or the name on the cheque (if paying by cheque).
